Adherence Management (AdM) Process

BEARS Adherence Management (AdM) "pronounced ADAM" Coaching® is the process of promoting patient adherence thereby reducing hospital readmissions. This is done by following the patient's progress, post discharge and positively reinforcing the behavioral changes advised in their plan of care. Everything from medication adherence to physical therapies and diet changes will show positive results. AdM Coaches® continue the work of patient adherence where everything else is left at the hospital doors, post discharge.

Our comprehensive process starts at admission and follows the patient throughout their plan of care to help promote adherence and produce in your patients healthy lifelong habits.
